これはいい論文と思った。読んでください。
頭の中で起こっていることまで将来的には解明できたらすばらしいです。その昔、研修医の時にベンゾジアゼピンを長期服用して耐性の出来た人にフルマゼニールを投与するとその耐性がリセットさせるという論文を読んだことがあります。この論文の患者さんでもそのようなことが起こるのでしょうか？

脳と心
Wired News: Clear Pictures of How We Think
functinal magnetic-resonance imaging(fMRI)の話です。
まあそうなんだろうけど、結局偉いのは器械を作った人ではないかと言う気もします。
この文章では心と脳の問題を解く鍵がfMRIにはあるという趣旨なのですがそれはそうかも知れません。何せ人間の脳に電極を埋め込むわけにはいかなのでしょうがないです。

これはおもしろい
Abstract:
In vitro studies suggest a role for c-Jun N-terminal kinases (JNKs) in proatherogenic cellular processes. We show that atherosclerosis-prone ApoE–/– mice simultaneously lacking JNK2 (ApoE–/– JNK2–/– mice), but not ApoE–/– JNK1–/– mice, developed less atherosclerosis than do ApoE–/– mice. Pharmacological inhibition of JNK activity efficiently reduced plaque formation. Macrophages lacking JNK2 displayed suppressed foam cell formation caused by defective uptake and degradation of modified lipoproteins and showed increased amounts of the modified lipoprotein-binding and -internalizing scavenger receptor A (SR-A), whose phosphorylation was markedly decreased. Macrophage-restricted deletion of JNK2 was sufficient to decrease atherogenesis. Thus, JNK2-dependent phosphorylation of SR-A promotes uptake of lipids in macrophages, thereby regulating foam cell formation, a critical step in atherogenesis.

何でこんな本を書いたのでしょうか
いや～、面白い
星ひとつ、というよりも、「分類」的評価はＤかＦ
帰宅途中で立ち読みして結局買って帰りの電車で読みました。
これはためになる本です。
著者は文学研究者で自然科学者ではないのですが、とても論旨のしっかりした本です。
特に第一章と二章の部分はよい。臨床でも基礎でも”評論家”のような人はいくらでもいる。
一回立ち読みでもいいし図書館でかりてもいいが読んだ方がいいと思います。最も値段は760円ですけど。
第三章の学者分類もなかなかおもしろいです。
A.文句なしに偉大な学者
B.ちょっと怪しいけれど、まあ偉い学者
G.はしにも棒にも掛からない、ろくな業績もない、ただ大学教授であるだけという学者
C.マスコミ的には無名だが、堅実な研究論文を着実に書いている人
D.「評論家」として優れていてマスコミ的にも、学者としてどうかというと疑問符が付く
F.元は学者だったんだろうが、いつしかいつしか一般向けエッセイを量産する、あるいはテレビタレントのようになってしまったひと
E.マスコミ的に有名だが、その学問はインチキである
でそれぞれ例が引いてあります。
麻酔学会の世界でも同じ分類はできますね。だれがどれだとは言わんけど。

税金でやってる癖に勝手なことするな という話

米国の医学研究を含んだ健康維持、増進のために国家から出される研究費はNatinal Institutes of Health(NIH)という機関がregurateしています。 director(長官)が交替するたびに結構な方針転換がなされます。
少し前に”prevent them(intramural scientist of NIH) to be paid to advice or speak at institutions that receive funds from NIH"という方針が打ち出されました。例えばNIHの科学者が自動車で一時間もかからないJohns Hopkins Univ,でセミナーをしたとしても謝礼を受け取ってはいけないということいなります。もとより私企業との関係は厳しく規制されています。
上の記事はこの方針転換にたいして NIHの研究者の内170人の人が異議を唱えた要望書を提出したという話です。
国家予算をつかって行われた研究の成果を公開することは予算の出所を考えたら当たり前でありそれで何らかの付加的な利益を得るのはおかしいという考えです。お金をもらいたければ私的な研究費を用いて研究をしてそれで利益を上げればいいという至極当然な考えのように思えます。
かつての国立大学はいまや独立法人化して身分も公務員ではなくなりました。しかし運営交付金という形で現在もほぼ100%というより大学病院の赤字も含めたら100%以上の補助を政府からつまり税金から受けています。
実は日本でもこのような規則はあります。ぼくの以前の職場は独立行政法人の研究所でしたが国立大学でセミナーを行う場合でも謝礼を受け取ることが禁止されていました。交通費とか滞在費は実費にかぎり受け取りが許可されます。
大学にいる人でこんなこときちんと守っている人はどれくらいの比率でいるのでしょうか。 しっかり調べると職務規定違反だったりしますよきっと。

NOによるカルシウム濃度制御
S-Glutathiolation by peroxynitrite activates SERCA during arterial relaxation by nitric oxide - Nature Medicine
これはすごいね。
要するにNOの血管弛緩作用はcGMPとかを考えなくともいいと言うことを言っているのではないだろうか。
また亜硝酸製剤による血管の弛緩もNOがそのものとして関与している必要は無いということになる。
Nitric oxide (NO) physiologically stimulates the sarco/endoplasmic reticulum calcium (Ca2+) ATPase (SERCA) to decrease intracellular Ca2+ concentration and relax cardiac, skeletal and vascular smooth muscle. Here, we show that NO-derived peroxynitrite (ONOO-) directly increases SERCA activity by S-glutathiolation and that this modification of SERCA is blocked by irreversible oxidation of the relevant cysteine thiols during atherosclerosis. Purified SERCA was S-glutathiolated by ONOO- and the increase in Ca2+-uptake activity of SERCA reconstituted in phospholipid vesicles required the presence of glutathione. Mutation of the SERCA-reactive Cys674 to serine abolished these effects. Because superoxide scavengers decreased S-glutathiolation of SERCA and arterial relaxation by NO, ONOO- is implicated as the intracellular mediator. NO-dependent relaxation as well as S-glutathiolation and activation of SERCA were decreased by atherosclerosis and Cys674 was found to be oxidized to sulfonic acid. Thus, irreversible oxidation of key thiol(s) in disease impairs NO-induced relaxation by preventing reversible S-glutathiolation and activation of SERCA by NO/ONOO-.

FEBS Letters The intravenous anesthetic propofol inhibits hypoxia-inducible factor 1 activity in an oxygen tension-dependent manner
In pressだった高淵先生の論文が正式に出ました。
Abstract
Hypoxia elicits a wide range of responses that occur at different organizational levels in the body. Hypoxia is not only a signal for energy conservation and metabolic change, but triggers expression of a select set of genes. The transcription factor hypoxia-inducible factor 1 (HIF-1) is now appreciated to be a master factor of the gene induction. Although knowledge on molecular mechanisms of HIF-1 activation in response to hypoxia is accumulating, the molecular mechanism of maintenance of HIF-1 activity under normoxic conditions remains to be elucidated. We demonstrate that the intravenous anesthetic propofol reversibly inhibits HIF-1 activity and the gene expression mediated by HIF-1 by blocking the synthesis of the HIF-1α subunit under 20% or 5% O2 conditions, but not under 1% O2 conditions.

Challengingな論文
The lungs of newborn rats exposed to 60% oxygen for 14 days develop an injury that shares morphologic similarities to human bronchopulmonary dysplasia (BPD). Neutrophil influx into the lung, as part of an inflammatory response, may play a pivotal role in the development of BPD. A neutrophil chemokine, cytokine-induced neutrophil chemoattractant-1, which signals through the neutrophil CXC chemokine receptor-2, is increased in the lung tissue of newborn rats exposed to 60% oxygen. The purpose of this study was to explore the role of neutrophils in the rat model of BPD by inhibiting neutrophil influx using SB265610, a selective CXC chemokine receptor-2 antagonist. SB265610, administered to 60% oxygen–exposed newborn rats from birth to 14 days, completely inhibited neutrophil influx. It also attenuated increased production of reactive oxygen species in newborn rat lung tissue after exposure to 60% oxygen for 4 days. Lung morphometric analysis revealed that 60% oxygen for 14 days, when accompanied by treatment with SB265610 to prevent neutrophil accumulation, increased alveolar formation over that seen in newborn rats exposed to air. These data suggest that exposure of the neonatal lung to moderate hyperoxia may enhance postnatal lung growth, provided postnatal pulmonary inflammation is suppressed.

すでに現在の”バイオ”は原理を追究するだけでなく”特許”を独占し”金儲け”をすることが強調されているのでどんなささいな方法も”効率”よく目的を達成できたらそれが生き残っていく。

コバルトによる心臓のpreconditioning

まだ全文を検討していないがこれはかなり複雑。
Acute systemic hypoxia induces delayed cardioprotection against ischemia (I)-reperfusion (R) injury via inducible nitric oxide synthase (iNOS)-dependent mechanism. Because CoCl2 is known to elicit hypoxia-like responses, we hypothesized that this chemical would mimic the delayed preconditioning effect in the heart. Adult male mice were pretreated with CoCl2 or saline. The hearts were isolated 24 h later and subjected to 20 min of global I and 30 min of R in Langendorff mode. Myocardial infarct size (% of risk area; mean ± SE, n = 6–8/group) was reduced in mice pretreated with 30 mg/kg CoCl2 (16.1 ± 3.1% vs. 27.6 ± 3.3% with saline control; P < 0.05) without compromising postischemic cardiac function. Higher doses of CoCl2 failed to induce similar protection. Electrophoretic mobility gel shift assay demonstrated significant enhancement in DNA binding activity of hypoxia-inducible factor 1 (HIF-1) and activator protein 1 (AP-1) in nuclear extracts from CoCl2-treated hearts. Activation of HIF-1 and AP-1 was evident at 30 min and sustained for the next 4 h after CoCl2 injection. In contrast, CoCl2-induced protection was independent of NF-B activation because no DNA binding or p65 translocation was observed in nuclear extracts. Also, CoCl2-induced cardioprotection was preserved in p50 subunit NF-B-knockout (KO) mice (11.1 ± 3.0% vs. 25.1 ± 5.0% in saline-treated p50-KO mice; P < 0.05). The infarct-limiting effect of CoCl2 was absent in iNOS-KO mice (20.9 ± 3.0%). We conclude that in vivo administration of CoCl2 preconditions the heart against I/R injury. The delayed protective effect of CoCl2 is achieved through a distinctive signaling mechanism involving HIF-1, AP-1, and iNOS but independent of NF-B activation.

computerのkeyboardはHappy Hacking Keyboardがお薦めです。Lite2でもいいがprofessionalのキータッチはすばらしいと思う。少し小振りで手の小さいぼくでも指使いがなめらかに出来ます。結局手首というか肘というかが楽だし、肩も凝らないので快適。

一日30分程度30%O2を吸引したからといって糖尿病などがよくなるとはおもえないのだが。
詳しく見てみるとやっぱり少し変だと思います。このdata。多分雑誌に投稿したら取ってくれるところはどこもないと思います。まさに信じるものはすくわれるという原理に基づいた一種の"商法”ですね。

を使った臨床研究の論文がNew England J Medにでています。
NEJM -- Combination of Isosorbide Dinitrate and Hydralazine in Blacks with Heart Failure
黒人にのみ効果のあるtherapyです。
なので本来日本人にだけ効くとかいう話がいくらでもあっていいわけですよね。だから日本人向けの臨床治験もどんどんやったらいいと思います。

やっていることは極普通の解析で、さまざまな疾患の解析に利用されている方法です。それ自体問題はないと思います。問題はそれではこのような手法で”麻酔薬”の麻酔作用を説明するターゲットが見いだせるかと言うことだと思います。
今回はEtherの感受性に関する遺伝子ということですが、感受性の試験が結構あやしい。
また今なら、GABA受容体が怪しいなら四の五の言わんと遺伝子改変ネズミを作って見たらいいのではないかということがあります。

human papillomavirus (HPV)は子宮頚癌を引き起こすことが知られているウイルスです。HPVに対するワクチンをGlaxoSmithKlineが開発に成功し、臨床治験をしたところHPVの感染率が下がることが明らかになりました。
これが本当に子宮頚癌の発症を抑えることができたらすばらしいです。

日本医師会雑誌132巻第9号を読みました。
"補完・代替医療の現状と問題点"はなかなか読み応えのある特集です。
”なぜ日本の産婦人科専門医が減少しているのか？—九州大学の産婦人科医局に参加する医師の動向の変化”はおもしろいです。九大の医局員の動向を様々な角度から分析して最近の若者の動向について議論しています。麻酔科の場合、単一の医局の場合母数が小さくこのような解析が出来ないかもしれませんけど。これは必読です。

ニコチンがショック患者を救う
Cholinergic agonists inhibit HMGB1 release and improve survival in experimental sepsis - Nature Medicine
a7 nicotinic acetylcholine receptor (a7nAChR)の刺激でマクロファージからHMGB1が放出されるのが阻害されて実験的なseptic shockからのsurvival rateがあがるという話です。
そういえばちょっと前に嗅ぎタバコを吸引させるといいとかいう論文をみた。
少しいかがわしかったので読まなかったが今から考えるとコレと関係あるのかな。

ついに!!
Williams et al., 10.1126/science.1105010Hemoxygenase-2 is an Oxygen Sensor for a Calcium-Sensitive Potassium Channel
これはepock makingな論文です。自分で再現したわけではないですが論文を読む限りかなり信憑性は高いと思います。
例えば揮発性吸入麻酔薬や局所麻酔薬がこの酵素の活性をどう修飾するが直接assayすればいろんなことがわかると思います。
すごい論文です。
一応、abstractを追記に入れておきます。
ScienceのExpress電子版ですので図書館の購読契約では現時点では読むことが出来ないと思います。
ぼくは自分でScienceを取っているのでその購読権で読むことが出来ました。
Modulation of calcium-sensitive potassium (BK) channels by oxygen is important in several mammalian tissues, and in the carotid body is crucial to respiratory control. However, the identity of the oxygen sensor has not been known. We demonstrate that hemeoxygenase-2 (HO-2) is part of the BK channel complex and enhances channel activity in normoxia. Knock-down of HO-2 expression reduced channel activity, and carbon monoxide, a product of HO-2 activity, rescued this loss-of-function. Inhibition of BK channels by hypoxia was dependent on HO-2 expression and was augmented by HO-2 stimulation. Furthermore, carotid body cells demonstrated HO-2-dependent hypoxic BK channel inhibition indicating that HO-2 is an oxygen sensor that controls channel activity during oxygen deprivation.

今週の一押し 2004年第44週 "がん研究レース"
ワインバーグ博士は癌研究の最先端を走り続けている人。文章もうまくこの本以外にも数冊出版しています。ワトソン博士ほど皮肉屋でなくなかなかバランスのよい癌研究クロニクルとなっています。翻訳は京大の野田先生ご夫妻がなさっていますがこれもすばらしい。英語版とつきあわせて読んだことがありますが完璧な訳だと思います。
内容は少し古く４年ほど前までで終わっています。最近のepigenicのお話しは無しです。でも一般読者にはあまり関係ないと思います。

米国議会に医師がどれくらい存在するかを調べています。最近は率が減っているのだそうでそれが立法に何らかの影響を与えているかもという論旨です。

高月清先生が瑞宝重光章を受けられます。
ヒトにガンを作ることが証明されているウイルスは５つ現在までに知られています。HTLV-Iはそのうち唯一のレトロウイルスです。この研究無しには、AIDS/HIVの研究があれほど急激に進展しなかったことは明らかです。
またウイルスが原因のガンのうちATLほどウイルスとの関係が明確なガンはありません。つまりHTLV-I negativeであるATLはありません。またHTLV-Iは母乳の授乳で感染しますのでその経路をたてばHTLV-Iはそのうち実質的にはなくなることになります。
高月先生はご自分の存命中にATLという疾患概念を確立し、原因の同定、また根本的な予防法の確立まで見届けた非常に幸福な医者であるといえると思います。
またあまり認識されていませんがHTLV-Iのコードするトランス因子であるtaxの研究が、NF-kBの活性化の研究にブレイクスルーをもたらしました。
またこの一連の流れは日本医学が世界に誇る金字塔の一つと思います。
先生はすでに文化功労者でありますので後は文化勲章とノーベル賞だと思います。
しかし肩書きが熊本大学名誉教授というのは困りますね。現職の肩書きを使ってもらわんと。うちの病院の財団の理事をしていただいているのに、宣伝にならん。

この論文もノリはさっきの論文と似ている。今まで関連が指摘されていない分子と事象の関連を追及。組み合わせが新奇であればあるほどcatchyと言うわけです。KOでも感じの似た論文が多い。つぶしてみたら本命のphenotypeには差がなく、いろいろと探しているうちに”えっ”とか”あっ”と言うようなphenotypeにぶち当たりめでたくNatureとかそういうパターン。別に”おちょくって”いるわけでないですよ。念のため。
