近藤誠氏の巻頭言が象徴しているようにかなりの現状批判の論調です。
その中でも
日本の医学医療と生命倫理　谷田憲俊
とか
日本の医学と臨床現場　友岡憲彦
受診保障の医療経済学—患者自己負担をめぐって　馬場園明
には示唆に富む内容が多いような気がした。

しかし近藤先生
”のみならず，第二次世界大戦中には多数の臨時軍医を急造し，戦後その全てに医師免許を与えてしまった．彼らが日本医師会の，医療改革に反対するという横暴を支え，地盤を子女に譲るための金権私立医大を作らせたがために，低質医を拡大再生産する悪循環に陥った．”は言い過ぎでないか？

やりすぎか！?

虚血性心疾患患者で高位胸部硬膜外ブロックをしたら心筋への血流が改善するかどうかの検討。
C6/C7-Th8/11まで感覚をブロックすると少しは良いらしい。
でもここまでやるか。

二酸化硫黄が脳を守る

これはおもしろいよ。
二酸化硫黄を吸入すると虚血に強いマウスができる、らしい。
心肺中に低濃度で流すとかになったら臭くてしようがなさそう。

高校の教科書にでたらすごい

今年2005年はWorld Year of PHYSICS 2005だそうです。
Einsteinが1905年に、光電効果、特殊相対性理論、Brown運動の概念を一度に提出してから100年を記念してのことだそうです。
この三つの理論、よく考えたら高校の物理の教科書にも載ってい（少なくともぼくのときは載っていた）ますよね。凄いです。
ぼくもつねずね自分の発見したことが教科書に出るようになるような研究をしたいものだと思っていますが大学レベルの教科書には何とかとどいたかもということはあるのですが高校の教科書にはまだ届いていないと思います。高校はては中学（Brown運動は中学の教科書にも載っていたような気がする）の教科書に出るような基本的な発見を記述したいものです。

これはすごい。よくこんなこと思いついたなと感心します。しかし機序はどんなものなのだろう。今家で論文の全文を読めないのでなんともいえないのだが..
武藤先生もこんなことなさっているのですね。びっくり。COX-2/HIF-1のaxisはとくに驚くことはないが重力が関与するのはすごいやね。

オープンアクセスの雑誌が花盛り

Wired News: Open-Access Journals Flourish
オープンアクセスの雑誌が花盛りと訳すのでしょうか。
open accessであるというのとどんな雑誌でも載るというのは違う。例えばPublic Library of Scienceはopen accessだが掲載されるのはもしかしたらNatureよりも難しいかも。
またcritical care |の様なタイプのやつは一応refreeもいるpeer reviewed journalだがはっきり言って内容に乏しいような気もする。でもこういうのに出ても一報は一報。
難しいですね。

やっぱりお墨付きがあると安心しますね。
NYTにもReuters伝で報告されています。

この研究の研究費はNHLBIが支出していてdirectorのコメントも出ています。これは重要なことで要するに国家予算で研究を進めてこれにより喘息の維持治療のあらたな考え方が生まれるかも知れないわけです。

底知れなく広がっていきますね。
こうなるとHIFの活性にしても関わるすべての分子の動向を記述しないと正確に言えなくなるわけで有る薬剤がHIFを阻害するまたは活性かするとしてもすべての分子に対しての作用を調べないと正確なことは言えないことになる。しかも未知の分子が山ほど有るわけでなかなか一筋縄ではいかない。
ここまで複雑になるともう良いかと言う気にもなってくるな、実際。

Hsp20っていままで知りませんでした。
しかしこの分野登場人物が多すぎますね。
どれが重要なのか階層も定かでないし、例えばcardioprotectionの作用をもつ物質を同定しても作用機序を明らかにするためにはすべての分子の動きを記述しないと科学的には完全でないということになりますからしんどいです。

どこのコーヒーがうまいか

雑誌Brutusの2005/3/15号の特集は
"COFFEE AND CIGARETTES"でした。
今日、図書館で借りて読んでみておもしろかったのでこのblogのテーマと違うのですが紹介します。

おもしろかったのはtake outというかいわゆるチェーン店のコーヒー屋さんとかファーストフード屋のコーヒーの採点結果です。
23店のコーヒーの中で
#1,Seattle's Best Coffee
#2, NEW YORKER'S Cafe／ニューヨーカーズカフェ
#3, ロッテリア｜LOTTERIA
という結果。３位のロッテリアが意外にも健闘。しかも安い。
ただし例えばスタバとかはアメリカーノが評価対象なのでご注意を。

運動したらHIFが活性化する

運動すると筋肉でHIFが活性化するのだが、VHLの発現が落ちていくのがその機序らしい。

Opioid receptors stimulation do not affect the cellular hypoxia-induced gene responses mediated by hypoxia-inducible factor 1 in cultured cell lines

Abstract
Maintenance of oxygen supply to meet demand is an essence for patient management in operation theaters and intensive care units. Hypoxia (reduced oxygen availability) induces a series of adaptive physiological responses including gene inductions. Hypoxia-inducible factor 1 (HIF-1) is a master transcription factor regulating hypoxia-induced gene expression to maintain homeostasis in living body. We previously reported the effects of the volatile anesthetic halothane, the intravenous anesthetic propofol, and the local anesthetics on the HIF-1 activation. Opioids are potent analgesic agents, which are widely used in clinical fields. Therefore, we investigated the effect of opioids on the HIF-1 activity. SH-SY5Y human neuronal cells, which express opioid receptors intrinsically, are cultured under 1% or 20% O2 conditions with or without treatment by either of DAGO, DPDPE, and U-50488, which are the selective agonists of μ-, δ-, and κ-opioid receptors, respectively. Expression of subunits of HIF-1, HIF-1α and HIF-1β, were examined by Western blot using the specific antibodies. Expression of mRNA of HIF-1 and the HIF-1-dependent gene were investigated by reporter assay. Neither of the selective agonists of opioid receptors tested affected the HIF-1 activation by hypoxia. The selective agonists of opioid receptors do not affect the HIF-1-dependent cellular hypoxia-induced gene responses.

大学からmedlineにつながりません。というか外国に出て行かない。
夕方は、大学の外に出て行かなかった。
